Please read these Terms of Service (“Terms”) carefully before using optimalstatehealth.com (the “Website”) or our services. By accessing or using the Website, you agree to these Terms. If you do not agree, please do not use the Website.
About our services
Optimal State Health is a physician-led telehealth medical practice owned and operated by Dr. Ahmad Ibrahim, DO. We provide hormone, metabolic, and wellness care — including testosterone replacement therapy, peptide therapy, medical weight loss, libido and sexual health care, and longevity care — to patients located in California.
This Website is informational. Clinical services, including consultations, prescriptions, lab orders, and ongoing care, are delivered through our HIPAA-compliant patient portal (CharmHealth) and are also governed by separate clinical agreements and informed consent forms. If those documents conflict with these Terms on a clinical matter, the clinical documents control.
No doctor-patient relationship from website use
Browsing this Website, reading its content, booking a consultation, or submitting a contact form does not by itself create a doctor-patient relationship. That relationship begins only after you complete intake, sign the required consent forms, and are accepted as a patient through our clinical platform.
Not for emergencies
Our services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Messages and portal communications are not monitored for urgent issues around the clock, and Optimal State Health does not provide emergency or urgent care.
Telehealth services
Care is delivered by telehealth, primarily through secure video visits and portal messaging. Before treatment, you will be asked to review and accept our Telehealth Informed Consent, which explains the nature, benefits, risks, and limitations of telehealth care.
Eligibility
To receive clinical services, you must:
- Be at least 18 years old
- Be physically located in California at the time of each visit (we are currently licensed to practice in California only)
- Have a device with a camera, microphone, and reliable internet connection for video visits
- Provide accurate identification when requested
Anyone may access the Website for informational purposes.
Medical judgment and treatment
Booking a consultation does not guarantee that any treatment or prescription will be provided. All treatment decisions are made by Dr. Ibrahim based on your history, labs, and his independent clinical judgment. Treatment may be declined, adjusted, paused, or discontinued if it is not clinically appropriate or safe.
Some medications we may prescribe are compounded by state-licensed pharmacies. Compounded medications are not FDA-approved and have not been reviewed by the FDA for safety, effectiveness, or quality. Some uses of medications may be off-label. These will be discussed with you before you start.
Your responsibilities
As a patient, you agree to:
- Provide complete, accurate, and up-to-date health information, including medications, supplements, and medical conditions
- Complete lab work and follow-up visits recommended for safe monitoring of your treatment
- Use medications only as prescribed, and not share them with anyone else
- Tell us promptly about side effects, new symptoms, or changes in your health
- Keep your portal login secure and not allow others to use your account
Continued treatment may depend on completing required monitoring. For your safety, prescriptions may be paused if labs or follow-ups are overdue.
Prescriptions, pharmacies, and medications
Prescriptions may be sent to a pharmacy of your choice or, when appropriate, to a partner pharmacy that ships directly to you. Pharmacies are independent of Optimal State Health and are responsible for dispensing, shipping, and their own pricing and policies. We are not responsible for pharmacy delays, shortages, or shipping issues, although we will help where we can.
Medications and supplies dispensed to you generally cannot be returned or refunded once shipped, consistent with pharmacy regulations.
Appointments and cancellations
- Cancellation notice: please give at least 24 hours' notice to cancel or reschedule an appointment.
- Late cancellations and no-shows: appointments canceled with less than 24 hours' notice, or missed without notice, may be subject to a fee.
- Late arrivals: if you join a video visit more than 10 minutes late, the visit may need to be rescheduled.
Pricing and payment
- Your initial consultation is free. There is no obligation to start treatment afterward.
- You approve your price first. Plans are personalized. Before treatment begins, you will be told the exact price of your plan, and nothing is started or charged until you agree to it. Depending on your plan, labs and medications may or may not be included.
- Changes to your plan. If your plan changes — for example, a new dose or an added therapy — you will be told the new price and must approve it before it takes effect.
- General price changes. If we change pricing for an existing membership, active members will be notified at least 30 days in advance.
- How payment works. Payments are processed securely by Stripe through our patient portal (CharmHealth), not on this Website. Memberships are billed monthly. By approving your plan, you authorize recurring charges until you cancel.
- Insurance. Our membership is not billed to insurance. Some labs or medications may be eligible for insurance coverage depending on your plan, but coverage is not guaranteed.
Membership cancellation
- You may cancel anytime by emailing support@optimalstatehealth.com or messaging us through the patient portal.
- Cancellation takes effect at the end of your current billing cycle. Fees already paid for the current period are not refunded.
- There are no cancellation fees, termination penalties, or long-term contracts.
- If you cancel while on treatment, we will help you understand how to stop or transition safely.
